Case Study: Supporting Neurological Recovery with Pilates

Rebuilding Strength & Balance with Pilates After Spinal Cord Injury

When Ruby first began working with me, she was navigating the complex recovery from spinal cord injury and concussion. Her symptoms included spasticity in her right arm and left leg, tingling extremities, muscle weakness, limited flexibility, and a forward thrust of the head. Daily movement felt unstable and stiff—especially in the spine, hips, and legs—and her nervous system was often overwhelmed.

Our Approach

Together, we focused on three core goals:

  • Improving postural awareness
  • Rebuilding Strength and Balance
  • Calming the nervous system through breath-led movement

To do this, we worked with the Fletcher Pilates® fundamentals, blending classical mat exercises with targeted balance work at the barre and on the floor. I emphasized symmetry through side-to-side movement, neuromuscular integration, and careful attention to alignment—especially through the head, neck, and spine.

1. Improving Postural Awareness
One of our first priorities was helping Ruby feel more balanced and upright in her body. Through Pilates exercises that emphasized core activation, head and spine alignment, and foot-to-hip connection, she began to notice where her posture was collapsing and how to gently correct it.

2. Rebuilding Strength and Balance
As Ruby grew stronger, we introduced standing Pilates exercises to improve leg strength, ankle stability, and balance. We also worked on spinal alignment and posture, helping her feel more upright and in control throughout the day. These movements not only built physical strength—they improved her coordination and body awareness.

3. Breathwork to Calm the Nervous System
Each session incorporated breath-led movement. This approach helped calm Ruby’s nervous system and reduce spasticity. Deep, rhythmic breathing brought a sense of focus and ease, turning the session into a moving meditation that supported both physical and emotional regulation.

The Results

After just 4.5 months, Ruby had made profound progress. She’s regained spatial awareness, experienced more fluid movement, and reported stronger, more coordinated limbs. She can now use breath and intentional movement to interrupt spasms and maintain better alignment—especially in her neck and spine. Her hip mobility has improved, leading to noticeable changes in her feet and posture.
